Informed Consent

1. Purpose of the Survey: The purpose of this video survey is to gather insights into the safety challenges and intervention opportunities within the psychedelic ecosystem. Your responses will inform strategic processes, including shaping the content and discussions at PSI’s Psychedelic Safety Summit, and contribute to the development of reports, white papers, and other materials related to psychedelic safety and public health.

2. Use of Information: The information you provide will be used to:

   1. Inform Psychedelic Safety Institute’s Psychedelic Safety Summit agenda and related strategic processes.

   2. Develop theories of change, gap analyses, and strategic roadmaps.

   3. Produce content, such as reports and white papers, that may include anonymized quotes from your responses.

   4. Create educational and promotional video content (only with explicit additional consent)

3. Use of Likeness: Your likeness, including clippings from video recordings, will not be used without your explicit consent. If we wish to use any video clips featuring you (e.g., for a video montage), we will contact you directly to seek your approval. Without such consent, your likeness will not be included in any public materials.

4. Compensation: Upon completion of the video survey, you may optionally receive a compensation of $100 as appreciation for your time and effort. You may waive this compensation at your own discretion. Your choice to accept or decline compensation will not affect your participation in any way.

5. Non-Commercial Use: The information you provide will not be sold for commercial purposes. 

6. Data Collection and Storage: All survey data, including video recordings, will be securely collected and stored using encrypted databases. Data in transit and at rest will be protected using industry-standard encryption to ensure privacy and security. Only PSI staff and consultants covered under a confidentiality agreement will have access to this data. External parties will not have access to raw or identifiable data.

7. Confidentiality: Your responses will be treated confidentially. By completing this survey, you agree PSI may: a) Include your response as part of our aggregate data analysis, b) Use anonymous quotes without identifying information, c) Share aggregate findings with summit participants, d) publicly disclose that you completed the survey as part of our survey completion records. We will always obtain your explicit written consent before: a) Using your name or organization, b) Including identifiable quotes, c) Sharing any video or audio recordings

8. Risks and Benefits of Participation: There are no known risks associated with participating in this survey. The primary benefit of participation is contributing to the development of strategies that aim to improve public health and safety within the psychedelic ecosystem.

9. Voluntary Participation: Your participation in this video survey is entirely voluntary. You may decline to answer any question or withdraw from the survey at any time.

10. Duration of Participation: Completing the video survey is expected to take approximately 60-75 minutes.
